I
respectfully ask your assistance in obtaining answers to the following
questions regarding your relationship with the U.S. Vets Inc. and your
financial disclosure with the City and County of Honolulu's City Clerk's
office and the Hawaii State Ethics office. More specifically, I am
interested in your role as board member and employee of the U. S.
Veterans Inc:
Are you currently an employee of the U.S. Vets Inc? If yes, when was your original date of employment with the U.S. Vets? If you are no longer employed with the U. S. Vets. Inc. What was final date of employment with the U.S. Vets Inc?
A number of sources informed me that you maintained an office at the U.S. Vets Inc's. office in Kalaeloa. Did you ever maintain an office at U.S. Vets Inc. office in Kalaeloa? If yes, were you operating and conducting RTY Imports and your Kymberly Pine Consulting business from that office? If yes, were you charged rent by the U.S. Vets Inc.? If yes, how much did you pay? If no did you report this use of the space as a gift?
A review of your financial disclosure forms you indicated that you were a board member with the U.S. Vets Inc. Are you currently serving as a board member with the U.S. Vets Inc. If yes, what was the starting date? If no, what was the termination date?
On
your financial disclosure with the City and County of Honolulu's City
Clerk's office you indicated you are the owner of a business " RTY
Imports" but did not report owning that business to the Hawaii State Ethics office. When did you start owning the business "RTY Imports"? Why was this business not reported on the Hawaii State Ethics office disclosure forms you have filed?
Is the business RTY Imports registered with the Hawaii State Department of Commerce and Consumer Affairs? If no, Why not? (note , a search found no records of either business being registered).
In
a May 29, 2012, financial disclosure form that you filed with the
Hawaii State Ethics office you indicated that you were the owner and
sole propepritor of the business "Kymberly Pine Consulting". Is the business Kymberly Pine Consulting registered with the Hawaii State Department of Commerce and Consumer Affairs? If no, why not?
In your December 6, 2012, financial disclosure to the City and County of Honolulu's City Clerk's office you reported you were self employed as a "Consultant" earning one to nine thousand dollars. Is the consulting services you are providing done as Kymberly Pine Consulting?
Do you have any role in the operation of the U.S. VETS/YWCA's Transitional Housing for Women Veterans? If yes what is your role?
Have
any clients from the U.S. VETS/YWCA's Transitional Housing for Women
Veterans contacted you in your role as a City Council member asking for
assistance in their complaints against the U.S. VETS/YWCA, Daryl
Vincent, his staff , or Veteran Administration personnel for harassment, discrimination, racism, disparate treatment or eviction? If yes, did you inform the clients of your relationship with the U.S. VETS/YWCA? Did you recuse yourself? Did you refer them to an independent party? If yes, who?
As
a board member of the U.S. Vets Inc. have you participated in any
investigations regarding complaints against the U.S. VETS/YWCA, Daryl
Vincent or his staff?
Do you believe that your involvement as a board member is a conflict of interest? If yes, what are you doing to prevent or avoid this conflict?
A client filed claims that the U.S. VETS/YWCA, Darryl Vincent, his staff fed them meat and soft drinks that was four to ten
months old, mishandled their medical records, forced clients to work at
a church against their will, threatened to retaliate against a client
for being a whistle blower and more. As a board member/employee , and City CouncilMember what did you do to assist the clients of the program?
As a U.S. VETS board member did you or have you reviewed or become familiar with the clients' complaints?
As
a former State House Representative did you lobby or solicit the
acquisition of Federal, State , City or private funding for the U.S.
VETS/YWCA?
Did you provide support to the U.S. VETS in obtaining grants from the Leeward Coast Community Benefits Program (LCCBP)? As a board member of the U.S. VETS were you informed that money for the years 2007,2008 and 2009 lapsed to the City's general fund because they were not expended and accounted for by June 30, 2011?
I
am informed by Ms. Tina Quizon that you provided the following
statement to her regarding her complaints against the U.S. VETS/YWCA and
the United States Veteran Administration : "I have spoken to the U.S.
Vets. You will not be evicted today. I am in support of an investigation of your issue, which I understand U.S. VETS has agreed to." Are you privy to this information because of your status as a board member/employee of the U.S. VETS? There are at least three other investigations of Ms. Quizon's complaints. What is the status those investigations?
